Can't remember exactly if the provisional was just a card, or if it was card and paper, but if it is both... Assuming it is just the card you lost Tell them you lost both, as if not, they get you to send in your remaining part, before issuing 2 new bits. If you say you lost both, they immediately send you out 2 new bits, so you can then just ditch the old paper bit.
